Where can I find my portal invitation?
It may be in the spam folder. Please note the sender is TherapyAppointment.com rather than Four Points. If you're unable to locate it, please email info@fourpointscc.com and we'll resend the link.
If you're scheduled for a consultation, an invitation hasn't been sent yet. If you decide to move forward with an intake appointment, a link to register will be sent at that time.
The portal kicks me out. What's going on?
If you're logged out repeatedly, this is likely because more than one portal window is open at once. Please ensure you are logged in from only one window. Next, try clearing your cache/cookies. If the issue persists, please email portal@fourpointscc.com.
Where do I go to complete intake paperwork?
Intake documents are located in your portal under My Docs & Forms. Please complete intake forms prior to your first appointment.
How do I remove a credit card from my profile?
You can delete one of multiple cards by logging into the portal and going to "My Profile." If there is only one card on file, the platform requires that another card be added first, or for the card to be removed administratively (we apologize for the inconvenience). Please email us at info@fourpointscc.com and we'll be happy to take care of this for you.
