Out of Network Benefits & Private Pay Rates

Private pay rates vary by therapist. Rate information is available by contacting info@fourpointscc.com.​ Sliding scale rates are based on availability.

If you decide to see a therapist that is out-of-network with your health plan, we can provide you with a superbill upon request. Please let us know if you plan to pursue using out-of-network health benefits.

Good Faith Estimate

Those who elect to work with a non-participating provider (out of network) and pay privately for services are entitled to a good faith estimate. This is an explanation of the estimated cost for health care services. A good faith estimate is provided upon initiating services and upon request at any time.
