Private pay rates vary depending on the therapist and are available by contacting our team. Sliding scale rates may also be offered based on therapist availability.
If your therapist is out of network with your insurance plan, we'll provide you with a Superbill which can be filed with your carrier if you have out of network benefits.
Good Faith Estimate (GFE)
Those who do not have health insurance coverage or who opt to pay privately are entitled to a good faith estimate. This is an explanation of the total estimated cost for healthcare services, which is provided before receiving those services. A good faith estimate is available upon request, or will be initiated upon scheduling services.

Insurance FAQ
Information regarding benefits and payment responsibility are provided as a courtesy and intended to serve as an estimate only. A discrepancy may exist between information provided by Four Points and actual coverage and payment responsibility. We encourage all clients to independently verify coverage before receiving services by contacting the insurance company directly.
How will I know the cost of services and the amount insurance will pay?
Before the initial appointment, your coverage and payment responsibility will be verified. We'll let you know your anticipated payment amount through our secure client portal.
What forms of payment are accepted?
The preferred method of payment is credit card, however it is possible to make payment by cash or check for those who attend sessions in person. For those exclusively attending sessions via telehealth, credit card is the only available method of payment.
Are HSA funds accepted?
Yes, if funds are available in the form of a debit card, it can be used just like a credit card. Otherwise, some plans disburse HSA funds during claims processing. If you have questions about whether you can use your HSA at Four Points, please reach out.
