Questions
Four Points is an entirely therapist-operated practice. We pride ourselves on being accessible and responsive to clients including for matters that lie outside the therapy session, such as insurance and billing-related needs.
If you have questions, you can reach a member of our team by calling (970) 682-1337 or emailing info@fourpointscc.com. If you call our office, you may need to leave a message, but please be assured we'll return your call promptly.
Established clients may also use the Support Request on the Forms & Resources page.
Benefits Verification
During scheduling, insurance information is collected in order to verify benefits. Before the intake appointment, benefit and payment information is shared via the client portal.
Claims Submission
Claims are submitted electronically with our administrative team managing insurance matters behind the scenes. You are kept apprised whenever possible of changes that impact session fees, such as a deductible resetting.
Client Responsibility
Our usual practice is to accept credit card payments but cash and checks can be accepted for in person appointments. For those exclusively receiving teletherapy services, credit card is the only available method of payment.
