Private Pay & Out of Network Services 

Private pay rates vary by therapist. Rate information is available by contacting our team at info@fourpointscc.com.​ Sliding scale rates are based on availability.

If you decide to see a therapist that is out-of-network with your health plan, we can provide you with a superbill upon request. Please let us know if you plan to pursue using out-of-network health benefits.

Good Faith Estimate (GFE)

Those who elect to work with a non-participating provider (out-of-network) and pay privately for services is entitled to a good faith estimate. This is an explanation of the estimated cost for health care services which is provided before receiving those services. A good faith estimate is provided upon initiating services and upon request at any time.

Health Plan FAQ

Information regarding health plan coverage (benefits) and payment responsibility is provided as a courtesy and is intended to serve as an estimate only.

 

A discrepancy may exist between information provided by Four Points and your actual coverage and the payment you owe. We encourage you to contact your insurance company and independently verify your coverage before receiving services with Four Points Counseling Center. â€‹

How will I know the cost of services and the amount insurance will pay?

Before the first appointment, your health plan coverage and anticipated payment responsibility is verified and communicated to you via email. 

What forms of payment are accepted?

We prefer that credit card is used whenever possible, however clients are able pay by cash or check as long as services are attended in person. For those exclusively attending sessions via telehealth, credit card is the only available method of payment.

Are HSA funds accepted?

HSA cards can be added to your account like any other credit card. Some HSA and FSA programs may direct you to make payment through a third party, such as Instamed.

Are claims submitted to my insurance?  Or do I need Superbills to do this on my own?

Four Points manages all claims and submits directly to insurance, with the exception of instances in which clients choose to see a therapist who is out-of-network with their health plan. In these cases, we are happy to provide Superbills for those who wish to pursue their out-of-network health benefits.
